When you submit our eligibility form, we collect the information you provide: your name, phone number, email address, county, your relationship to the person needing care, and whether the person needing care has Michigan Medicaid. If you call or email us, we may collect information you share during that communication.
We automatically collect limited technical data such as the page you arrived from, your device type, and general location, through standard web analytics and advertising measurement tools (including Google Ads conversion tracking).

- To contact you about eligibility for the Michigan Home Help Program and our services
- To help you enroll and coordinate care with the Michigan Department of Health and Human Services (MDHHS)
- To respond to your questions

Health information
The eligibility form asks whether the person needing care has Medicaid. This is used solely to determine program eligibility. We do not request detailed medical records through this website. Any health information you share is handled confidentially and used only to provide and coordinate care services.
How we share it
We do not sell your personal information. We share information only with: (a) MDHHS and related state agencies as required to enroll in the Home Help Program; (b) service providers who help us operate (such as Google for form storage and email); and (c) when required by law.
Data storage and security
Form submissions are stored in secured Google Workspace systems accessible only to authorized staff. We use industry-standard safeguards, but no method of transmission over the internet is 100% secure.
Your choices
- You may request access to, correction of, or deletion of your information by contacting us.
- You may opt out of communications at any time by telling us by phone or email.
- You can disable cookies in your browser; this may affect ad measurement but not site functionality.
Children
This website is not directed to children under 13 and we do not knowingly collect information from them.
